If you are confident that you will be submitting an application on January 15, 2008, you must fill out this form and return it to us no later than October 22, 2007. The LWCF application review requires that we visit each community that applies for funding to evaluate the project site, as well as existing recreation areas and facilities.

We have found that these site visits allow one-on-one consultation with project coordinators and that their applications typically score higher as a result. We encourage you to take advantage of a pre-award site visit prior to completing your application. Please use this form to notify us so that we can schedule a site visit and assist you in the application process.
